Lyme disease is the most common vector-borne disease in the United States, with approximately 476,000 people diagnosed and treated each year. Many patients with undiagnosed and untreated early Lyme disease will progress to disseminated Lyme disease. Disseminated Lyme disease is often treated in the acute care setting and may be difficult for providers to identify without the classic erythema migrans rash of early localized Lyme disease. This webinar will review presentations of disseminated Lyme disease in pediatric patients as well as appropriate diagnostics and treatment plans utilizing the 2020 Lyme disease guidelines.

Fever is a broad differential and is a top reason for pediatric visits in acute care settings. With infectious diseases emerging, global travel resuming, and malignancy and rheumatological disorders often presenting in childhood, the clinician must be knowledgeable about fever etiologies across childhood. In 2021, the AAP updated their clinical practice guideline on the febrile infant. This presentation will discuss the history of fever, review causes of fever based on age and body system, and review the latest evidence on the diagnosis of fever.

Children in foster care are a vulnerable population with unmet health care needs complicated by a history of trauma, making providing comprehensive care difficult for Advanced Practice Providers (APPs). Care for this population is often transient and crisis centered, and an estimated 80% of children in foster care having one or more psychiatric diagnoses. Despite this rate of psychiatric diagnoses, children in foster care are prescribed psychotropic medications at a disproportionately high rate that is more than twice that of Medicaid controls. Stimulants are the most prescribed medication class followed by atypical anti-psychotic medications. A better understanding of the common mental health diagnoses of children in foster care, common medication indications and their monitoring, and available evidence-based treatments are imperative for APPs to care for this population. APPs need to be strong advocates for this vulnerable population to reduce the risk of psychotropic medication overuse and improve overall health outcomes.

Before 2018, there were no U.S. Food and Drug Administration approved medications for managing seizures in Dravet syndrome (DS). Common agents used in the antiepileptic drug regimens of patients with DS included clobazam, valproic acid, topiramate, and levetiracetam, among others; however, these agents alone rarely provide adequate seizure control. Management of seizures in DS changed in recent years with the approval of cannabidiol and stiripentol in 2018 and fenfluramine in 2020. This continuing education article summarizes available efficacy and safety data involving cannabidiol, stiripentol, and fenfluramine and provides a practical review of dosing strategies, pharmacokinetics, and monitoring interventions relevant to their use. The gynecologic assessment in prepubertal children is an essential element of a thorough physical examination. It is not unusual for pediatric health care providers, including pediatric nurse practitioners (PNPs), to feel challenged by assessing for gynecologic signs and symptoms and performing a physical examination of the external genitalia in prepubertal girls (Bhoopatkar, Wearn, & Vnuk, 2017; Dabson, Magin, Heading, & Pond, 2014; McBain, Pullon, Garrett, & Hoare, 2016). The benefits of routine genital examinations have been discussed in the literature, including increasing patient and caregiver comfort with an examination, improved health care provider skill and confidence with examination and diagnostic assessment, providing a baseline for future examinations, and documentation of previously undiscovered anomalies (Johnson, 2002). This continuing education article will discuss genital examination preparation and techniques, normal prepubertal hymenal variants, common prepubertal gynecologic problems, and genital examination findings concerning abuse.
