Some infants may not be recognized to have ductal dependent cardiac disease until the ductus arteriosus is closing and the patient is showing signs of poor perfusion or hypoxia. Detection and management of ductal dependence is critical to stabilize these patients until corrective procedures or surgeries can be offered.

Overuse of Psychotropic Medications in Children and Youth in Foster Care

This continuing education activity was reviewed and updated in 2025 to ensure compliance with current clinical standards, updated instructional content and best practices in health care education. Children in foster care are a vulnerable population with unmet health care needs complicated by a history of trauma, making providing comprehensive care difficult for Advanced Practice Providers (APPs). Care for this population is often transient and crisis centered, and an estimated 80% of children in foster care having one or more psychiatric diagnoses. Despite this rate of psychiatric diagnoses, children in foster care are prescribed psychotropic medications at a disproportionately high rate that is more than twice that of Medicaid controls. Stimulants are the most prescribed medication class followed by atypical anti-psychotic medications. A better understanding of the common mental health diagnoses of children in foster care, common medication indications and their monitoring, and available evidence-based treatments are imperative for APPs to care for this population. APPs need to be strong advocates for this vulnerable population to reduce the risk of psychotropic medication overuse and improve overall health outcomes.

The physical, emotional, and social burden of AR/C in children highlights the need for accurate diagnosis with optimal treatment. This review provides practical information on the diagnosis and management of pediatric AR/C. Key features of the patient history and physical exam needed to diagnose seasonal and perennial AR/C are covered. Various AR/C treatment options are reviewed such as allergen avoidance, pharmacotherapies, and allergy immunotherapy (both subcutaneous injections and sublingual tablets) as well as their mode of action, side effects, and their role in guideline-recommended therapy. Of significant concern is the steep rise in congenital syphilis cases, which poses grave risks to maternal and neonatal health. This article provides an overview of the epidemiology, risk factors, clinical manifestations, screening methods, and treatment strategies for syphilis, with a specific focus on congenital transmission. Drawing upon current literature and guidelines, the paper underscores the importance of early detection and intervention to prevent adverse outcomes associated with congenital syphilis. Furthermore, the article highlights the challenges in syphilis diagnosis, including the interpretation of serological tests and the management of neurosyphilis. Recommendations for practice, including screening protocols and treatment regimens, are discussed in detail. The conclusion emphasizes the urgent need for collaborative efforts among healthcare providers, public health agencies, and community stakeholders to address the syphilis epidemic effectively. By adhering to evidence-based guidelines and leveraging available resources, clinicians can play a pivotal role in reducing the burden of syphilis and safeguarding the health of vulnerable populations, particularly pregnant women and infants.
